I. Brief
description of our public body:
A. Our purpose is to provide
materials and services for the
recreational, social, informational,
and educational needs of the
community.
B. The total amount of our
operating budget for the current
fiscal year is $
Funding sources are property and
personal property replacement taxes,
fines, charges, and donations.
C, The library's administrative
office is located at 432 E Locust
Street, Chatsworth, IL 60921.
D The following number of persons

E. The following organization
exercises control over our policies
and procedures: The Chatsworth
Township Board of Library Trustees
which meets monthly on the second
Wednesday of each month at 6:30 p.m.
at the library. Its members are:
president, vice-president,
secretary, treasurer and three
trustees.
F. We are required to report and be
answerable for our operations to:
Illinois State Library, Springfield,
Illinois with Jesse White being the
State Librarian.

II. You may
request the information and the
records available to the public in
the following manner:
A. Your request should be directed
to the Freedom of Information
Officer..Mary Fisher-Miller
B. You must specify the records
requested to be disclosed for
inspection of to be copied. If you
desire that any
record be certified, you must
specify which ones.
C. You must reimburse us our actual
costs for reproducing and certifying
(if requested) the records.
You will be charged twenty-five
cents per page for employee copied
records.
D. The FOIA officer will respond to
a written request within seven (7)
working days of sooner if
possible, An
extension of an additional seven (7)
working days may be necessary to
properly respond.
E. Records may be inspected of
copied. If inspected, an employee
must be present throughout
the inspection.
F. You may appeal the decision of
the FOIA officer to the Board of
Library Trustees.
G. The records will be available at
the Chatsworth Township Library
during library's open hours.
